NCD vs LCD — what do these terms mean?

NCD (National Coverage Determination)

National-level policy describing when Medicare may cover a service/test/item. Typically more standardized.

LCD (Local Coverage Determination)

Local policy created by Medicare Administrative Contractors (MACs). Can vary by contractor/region and may include specific documentation workflows.
How to choose:
• Start with NCD if you want the most general national policy language.
• Use LCD when local contractor rules may matter, or when a provider references an LCD number/version.
Glossary
MAC: Medicare Administrative Contractor (regional contractor that processes claims and can publish LCDs).
ID: CMS document identifier (NCD ID or LCD ID).
Version: Revision number for the document used by CMS.
Updated: Date shown in the CMS report list for the latest update.
